Pre-Hospital Pathfinder

The Pathfinder tool is intentionally crafted for utilization by both registered and non-registered healthcare professionals. It is founded on the Manchester Triage System (MTS) and is intricately designed to seamlessly integrate with a suite of tools across various domains, all rooted in the same system:

  1. Emergency Triage: Deployed in Emergency departments and Ambulance services.
  2. Telephone Triage and Advice: Applied in Ambulance Services and Urgent Care Centres.
  3. Nursing and Residential Triage: Utilized in Nursing and Residential Homes.
  4. Community Triage: Employed by Out of Hours and Telemedicine providers; Telecare and Pendant providers; First responder organizations; Community assessment services.
  5. SAFE Pathways: Applied by Community-based clinicians.

The six triage tools share common attributes:

  • Reductive Methodology: Identifying life-threatening illnesses and injuries as a top priority.
  • Clinical Risk Management: Globally employed to effectively manage patient flow during times of clinical demand exceeding capacity.
  • Discriminator Dictionary: Utilizes a common set of discriminators defined in a shared discriminator dictionary.

While upholding the core principles of the Manchester Triage System, the Pathfinder tool diverges from the traditional 53 MTS presentational charts. Instead, it comprises three charts: Medical, Trauma, and Mental Health.

Certain discriminators and definitions have been adjusted to accommodate both registered and non-registered healthcare professionals. The tool necessitates a level of clinical competence to ensure users possess a sound understanding of discriminators within their scope of practice.
